1. Introduction

CompaHealth ("CompaHealth," "we," "us," or "our") operates a waitlist and short survey designed to understand the challenges caregivers and patients face after a hospital discharge. This Privacy Policy explains what information we collect when you visit our website or complete our survey, how we use it, and the choices available to you.

This policy applies only to our website and survey. It does not yet cover any future CompaHealth product or service, which will be governed by an updated policy at that time.

By submitting information through our website or waitlist survey, you agree to the practices described in this policy.

2. Information We Collect

We collect the following information directly from you:

• Email address — collected so we can follow up with you.
• Waitlist responses — including your description of the challenges you or someone you care for experienced after a hospital discharge. This information is provided voluntarily by you and may include sensitive details about a hospital stay or discharge experience.

We also automatically collect limited technical information when you visit our website:

• Website analytics and advertising data — through Google Analytics and associated advertising pixels, which may collect information such as your IP address, browser type, device information, pages visited, and how you arrived at our site. We use this information to understand traffic to our website and to evaluate the performance of paid advertising.

We do not currently collect protected health information (PHI) as defined under HIPAA through this website or survey. If, after speaking with you, we help coordinate or schedule an appointment on a patient's behalf, that process is governed separately by a HIPAA authorization form that the patient signs at that time — not by this policy.

3. How We Use Your Information

We use the information we collect to:

• Review waitlist responses to identify individuals whose experiences closely match the problems CompaHealth is trying to solve;
• Contact you by email to learn more about your discharge experience, including follow-up conversations;
• Operate, maintain, and improve our website;
• Measure and optimize the performance of our paid advertising campaigns; and
• Communicate with you about CompaHealth, including updates about our product and service as it develops.

4. How We Share Your Information

We do not sell your personal information. We share information only with the following categories of third parties, each of whom processes data on our behalf:

• Tally — the platform we use to host and collect responses to our survey and email/contact capture form.
• Google Analytics — used for website analytics and advertising pixels to measure and optimize paid advertising performance.
• Google Fonts — used to serve the typefaces on our website. Google may receive your IP address when your browser requests them.

These providers may process your information subject to their own privacy policies and terms. We may also disclose information if required to do so by law, or to protect the rights, safety, or property of CompaHealth or others.

If we later help arrange a specific appointment or service on a patient's behalf, that separate HIPAA authorization form — not this policy — governs how that specific health information is shared.

5. Data Retention

We currently retain waitlist responses and contact information (email address) indefinitely. We may update this practice as CompaHealth's product and data practices mature, at which point this policy will be updated accordingly.

6. Your Choices & Rights

You may:

• Opt out of future communications from us at any time by replying to any email we send you or contacting us directly (see Section 9).
• Request deletion of your information by contacting us at the email address below. We will honor reasonable deletion requests to the extent feasible.
• Ask what information we have about you by contacting us directly.

Depending on where you live, you may have additional rights under applicable state or local privacy laws. Contact us and we will do our best to accommodate your request.

7. Cookies & Analytics

Our website uses cookies and similar tracking technologies (including Google Analytics and advertising pixels) to understand how visitors use our site and to measure advertising performance. You can control cookies through your browser settings; disabling cookies may affect your experience on our site.

8. Children's Privacy

Our landing page and survey are not directed at, and are not knowingly used to collect information from, individuals under the age of 18. If we learn that we have inadvertently collected information from someone under 18, we will delete it.

We take reasonable measures to protect the information you provide to us. However, no method of transmission or storage over the internet is completely secure, and we cannot guarantee absolute security or privacy.

10. Changes to This Policy

We may update this Privacy Policy as CompaHealth's product, data practices, or legal obligations evolve. We will update the "Effective Date" above when changes are made. Material changes will be communicated to you where appropriate.
